Our mission is to advance health equity for residents of Houston, Texas by addressing unmet social determinants of health needs. Through community-driven initiatives, we work to remove barriers and unblock pathways to optimal health.

WhoWe Are

We are a community-based, grassroots organization in Houston, Texas, dedicated to providing free community health support and case management for individuals and families who are indigent or under-resourced.

The systems that provide social and health services can be complex, and it’s not always easy to know where to begin or how to navigate multiple programs at once. Many people tell us that this process can feel overwhelming — especially when they are already facing challenging circumstances.

What We Do

We provide comprehensive, wraparound community health case management designed to make this process easier. Our team works alongside each client from start to finish, helping to:

  • identify needs and priorities

  • coordinate services across programs and providers

  • connect with appropriate resources

  • advocate and offer guidance throughout the process

Our Commitment

We believe that access, support, and clear guidance can make a meaningful difference. Our role is to walk with people through each step, helping them feel informed, supported, and hopeful about what comes next.
